Claim Forms

Settlement Information last updated on April 24, 2007.
The claim filing deadline is closed effective April 23, 2007. Claims sent after April 23, 2007 will not be accepted.

The Settlement is divided into two groups: Consumer Customers and Volume Licensing Customers. Select the group that applies to you.

  • Consumer Customers are purchasers who received their licenses for a covered Microsoft product by purchasing it from a retailer (either at a store, by mail, or the Internet) or who received their license for a covered Microsoft product with the purchase of a computer and do not qualify as Volume Licensing Customers.   • Standard Claim Form

    Online Standard Claim Form (5 Products or less only)

  • Volume Licensing Customers are purchasers who licensed multiple copies through Microsoft Open, Select, or Enterprise Agreement licensing plans. To obtain the Claim Form, please click the link below.
  • Volume Licensing Customers

If you experience technical difficulties downloading a claim form, please contact the Settlement Administrator at 1-800-572-0455 for assistance. Operators are available between the hours of 8 A.M. and 8 P.M. Central Time.

When downloading a claim form, it may be necessary to have your “Pop-Up Blocking” tool turned off. This will ensure that you will be able to view and download your claim form correctly. If you have a Pop-Up Blocker, it would appear as a button just below the address line.
