Welcome to the Humana Healthcare Provider Settlement (“Settlement” or “Proposed Settlement”) website established in connection with In re: Managed Care Litigation MDL No. 1334.

This website is intended to provide information to Class Members about the Settlement with Humana Inc. and its current and former direct and indirect subsidiaries and affiliates in the class action lawsuits known as Ashton, et al. v. Humana Inc., et al.; and Solomon, et al. v. Humana Inc., et al. (collectively referred to as the “Litigation”). As used on this website, capitalized terms have the meaning ascribed to them in the Settlement Agreement.

In order to obtain compensation under the Settlement, you need to submit no later than January 13, 2007 a Claim Form and estimate the total billed charges you have submitted to Humana for Covered Services to members of health plans insured or administered by Humana, its subsidiaries or affiliates. Class Members may receive a “base amount” of compensation under the Settlement even if they did not provide Covered Services to Humana members. For a complete list of Humana affiliates and subsidiaries, please refer to Exhibit 1.

As described more fully in the Question and Answer (Q & A) section of this website, Class Members were mailed a Notice explaining their rights under the Settlement as well as a Claim Form and Claim Form Instructions on how to fill out the Form. This website includes those mailed documents sent to Class Members. Accordingly, Class Members should check for updates to this website to ensure that they have received all relevant instructions.
